Special Measurement Order Forms

Special Measurement Orders, please order through the DoD EMALL web site.
The following forms are for manually submitting Special Measurement Orders via fax.
They are not to be used when using a credit card as your payment method.
You must submit a completed DD-1348-6 form along with a Special Measurement Form. Fax both forms to the Special Measurement Office at 215-737-3163.
NAVAIR Customers
ALL flight clothing custom fit requests must be submitted via NAVAIR Form 10126/1.
Fax this form along with a DD 1348-6 to NCTRF at 215-737-7277.
